Page Title not showing correctly

CMS 2.1.6 - Standard plugins, plus Captcha (0.5.3), FormBuilder (0.8.1.4), Gallery (2.2), LinkMgr (2.1.1). On an only slightly modified version of Simplex theme. using Pretty URLS.

I have noticed over the past few weeks that the Page title in the top of the browser shows " - Site Name" rather than "Page Title - Site Name".

The "You are here: Home» Page Title" gets stuck on the third page title. So all pages have the same page title showing. Home will have "You are here: Home» 3rd Title" and Contact Us will have "You are here: Home» 3rd Title"

So far I have tried,
Manually clearing the cache - no luck
Clearing cache in Admin - Works after a manual page refresh but after a few days, it returns.

Smarty and Page caching are enabled.
