formbuilder issue

Hi,

I'm using formbuilder, but since last relaese it is not possible to use the template for 'From Address', it is not visible.

I have downgraded to the formbiiulder version 0.8.1.1, because this was working on another site. But it seems not working with the latest CMSMS 2.1.2 core. (the fields are not displaying).
When upgrading to formbuilder 0.8.1.3 they are displaying, but then the template for 'From Address' it is not visible anymore.

Please can someone tell me what to do to make the template for 'From Address' in formbuilder 0.8.1.3.
Is there a work around, or is it possible to fix this?
