Form Builder problem: Email to User-Supplied Email Address

The "Email to User-Supplied Email Address" field does not seem to function correctly for me in Form Builder. Whenever I submit the form, I just get a single unpopulated bullet point above the form, and nothing gets sent.

I have attached the form xml to this message. I altered some content which contains personal information, but the form structure is the same.

Any ideas please?
