Help with FormBuilder

I have two forms that I need help with customization and also styling the form with css. Nothing too fancy though.

The first one is a copy of this form:

except without the advanced search function but if you can do it, it will be nice to have too.

Click the first radio button (Recommend a specialist for me), you will see the Speciality dropdown below changed based on your choice. If you choose the option "I will choose my doctor", a dropdown with list of doctor will appear.

Choose a Speciality in the first dropdown and the list of doctor in that Speciality will appear on the Doctor's dropdown. When you choose a doctor name, the doctor profile and schedule will appear below.

The Appointment time should be based on the doctor schedule i.e. Some doctor only practice on Mondays from 1300hrs to 1700hrs so you can only make appointment on a Monday between 1300-1500hrs or 1500hrs to 1700hrs.

The next one is a supplement order form. Beside the usual contact details which I will send you, the special requirement is similar to the first form but simpler.

I have 3 radio button options. A, B & C

Option A will open a text input field just for quantity.
Options B will open two input fields for Medicine Name and Quantity. Need to have an option for user to add more rows or can be fixed to 10 rows.
Option C will open up two input fields in a row for Quantity and a dropdown of month or day

If it possible, I like to release this customisation in the module download for others to use as well only if the you the developer agree to do so.
