FeedbackForm retains the 'Submit Form' button after form is submitted

Feedback Form Issue

CMS Made Simple 0.13, FeedBack Form 0.9.12, CMSMailer 1.73.9

Everything works fine, the submitted form gets emailed and written to the log file... I get a Thanks Message ... but there is still a Submit Form button under the Thanks Message...

I also get a duplicated Submit Form button (ie two of them) if there is an error in the form.... which is a tad confusing for me (and the user)
