Destination Shipping Module limited to one country.

I've completed my first e-commerce CMSMS site with the exception of shipping rates. I've installed the flat rate shipping module but it will only let me add one country.

Once I've added the first country the interface dissapears and I'm left with just the tab and a blank area below.

Any clues? Desperate to get this finished.

On a side issue is it possible to change the states drop down to counties or regions for us non US based sites?
