error 500 in fieldset CTLmodulemaker latest version

error 500 in fieldset CTLmodulemaker latest version

hi, I try to add a fields in a new module, created with CTLmodulemaker, but I receive the error 500 in the fieldset; and watching in firebug console, I see that the address http://www.mysite.it/amministrazione/mo ... inputs.php doesn't exist
I use the 1.9.1 cms version
