Gestbook no entries saved

Again I am new to this...

I have installed Guestbook ver 1.3.0 in my CMS ver 1.9 and I am not able to make entries in the guestbook from the form. If I make entries in the Admin gui everything works.
If I try to make an entry to the guestbook I get a version of my site with no template. (see attached image)
I have set the "Show message immediately" to enabled.

I have read about Restore the templates to default for the guestbook-templates and I have tried to do so. Two of the templates (Form template and Entry Template) don´t seem to work to restore to default. My browser cannot find the page if I try to restore theese two templates.

What have I done wrong?

Try to download and install newest module from here http://dev.cmsmadesimple.org/project/files/34#package-915
I have some problem when I install through CMS because there was older version.

I have tried to install the latest xml-version and the full download but I get the same result.
