Sharing your Formbuilder template

Hi just wondering if you all want to share and contribute your successful Formbuilder template layout?

That is not so easy to do since every form has different fields etc. but as far as styling them have you seen the sticky post at top of the is board?...

Yeah I already use this one.
But want to know everyone have other idea form.
Anyway why this template from Simon66 when using other align out template there is outline?
Only work on default template?
