Feedback form issue

I just started with cms made simple, I added the Feedbackform...
I ve created my form ... this one display correctly ... but when I click on the submit
button I've got another page with my layout + the submit button only(without the form)
I re-click on the submit, and I regot the button with ou the form...

I put an "Email Results to set Address" and I do not receive any mail... the CMSMailer work
