NEED HELP!!! DB tables not created

Trying to install CMSMS
On the step 6 I got

Creation additional_users table... [error]
...

The same for all tables

So the DB (MySQL 4.1.22) is empty.

Сonnection to the database looks to be OK

What can it be? What to look for?
