[solved] TinyMCE Table Support Intermittent

I have built many sites with CMSMS 1.5.4, and I keep running into the following issue.  I have enabled "Allow table operations" in the TinyMCE settings, but when editing pages the tables toolbar only shows up sometimes.  I can occasionally force it to show by hitting ctrl+f5 (full reload), but often the tables toolbar will just decide not to display.

I feel like this is probably caused by a conflict with my stylesheets, but I'm wondering if anyone else has experienced this and may have a solution.  I'm grateful for you responses.

Update: I am using Firefox 3.0.10, and this seems to only happen on pages that have images already in the content section.

Correction: it happens on any page, intermittently.  Pages that showed the toolbar fine before sometimes don't work.

SOLVED (potentially):  This is a weird fix and indicates to me that there are some resident issues here, but I seemed to have stopped the intermittent issues by enabling table operations also in the Frontend Toolbar Settings.  I'm guessing that what was happening is that sometimes the Frontend settings were loading instead of the backend settings.  Now that my settings for both match exactly, I seem to be getting the same result every time.
