Donation Form

I am not sure where to ask this question - so hopefully Product Support is the right place to start.

I would like to make a simple donation form - one field will use a drop down box to select where the donation where go to, the amount field will have the donator put in the amount he would like to donate and the other radio/drop down field will select if this is a one-time or recurring donation.

Reading thru the forum, I found this CMSMS users site (http://forum.cmsmadesimple.org/index.php/topic,29603.0.html), which is an excellent example of what I am looking for (just go to the donate page).

Is there a module for doing this?

Any help is much appreciated.
