[SOLVED] Quizzard Module - Will pay for more fields in user registration

Hi everybody

In the Quizzard Module I need more mandatory fields in the registration for the quiz taker?

I need additional mandatory fields for 'company_id' 'company_name' 'address' 'zipcode' 'city' and 'phone' in addition to the name and email fields that are already there.

The behaviour of the fields sould be as the name and email fields and they should beadded to th database and listed in the results view in the admin.
There should be a validation for 9 digits in the 'company_id' fireld and 4 digits in the 'zipcode' field.

As a 'nice to have' addition I would also like a mandatory checkbox similar to the email checkbox. It should have no other function than being checked before continuing to the quiz. As a 'do you accept the terms....' checkbox.

I will pay (PayPal?) for a functional customization of the module according to these spesifications. Name your price.

Since the response is low, I will clarify this:
When I say I will pay, I mean I will pay properly, at a fair rate for consumed hours or by fixed price, by PayPal or by bank transfer to (almost) any location in the world.
If you are a person with some credibility in this community, I will even pay some money up front, since nobody here really knows if I can be trusted to pay.
And I will claim no property to the finished product except for the right to use it on my site. So please - can somebody look into this?
