Creating a form in CMSMS which collects information of the person downloading

Hi CMSMS experts,

I wanted help in creating a simple form with about 5 - 6 fields which are very generic. When a user attempts to download a specific download, they should be redirected to a terms and conditions page which will have a tick box to accept.

Once the user clicks on accept -> it loads the form with 6 fields they need to fill in and then click on download.

When the user clicks on download it should provide them with a link to download the file. Or it should generate a URL which can expire automatically.

An email needs to be sent out to the sales team after completion of the process - so they can get in touch with the user who downloaded the file for invocing.

If anyone has a better solution to this, please feel free to suggest. Its very urgent and needs to be delivered as soon as possible.

WORK FLOW:
- User clicks on a file to download -> Terms and conditions page with Tick box to accept -> user enters in their details -> clicks on download button (submit) -> should bring up the file they wanted to download. -> a email/message sent through to sales team.
