FeedbackForm Module for Tell-a-friend Referal

I downloaded and installed the Feedback Form Module with CMSMS 0.10.3  and was trying to figure out if it can be used to create a "tell-a-friend" referral form. Has anyone used it in this way? If so, can you advise me on how to set up this form?
