Problems with forms (forms to go) and CMS

Guys the link below is to a thread on bebosoft's "Forms to Go" that I'm using to generate the "action" of a form I'm putting into a page on my website being powered by CMS.

Can anyone help me get a start with this?

I think that pasting the form code and the "final output" of the page that is generated is causing something to go hay wire, but I'm not sure...

Please, please, please help!
