Feedback Form Help - Arranging the fields in order

Issue: We are trying to arrange the form fields in a certain order:

First Name
Last Name
Email
Phone Number
Comment

However when we click on the UP ARROW or DOWN ARROW of any of these fields they do not move.

We can not seem to get them to move where we want them.

Does anyone know the bug fix for this or where we can go to arrange the order for these?
