Problems with formbuilder, new made formsnot showing

I am using cms1.1 and formbuilder 0.3

The forms that came with formbuilder like contact and sample form work fine and are shown on pages when I paste he link there,

But when I make a new form en place the form on a page I see nothing...

The only thing I have noticed is this error, which I sometimes see on the creation page of the new form:
