trying to upgrade but /install/upgrade.php results in 404 error

I'm trying to upgrade a CMSMS install from 1.0.2 to 1.0.7 incrementally and haven't gotten past 1.0.3.  I've downloaded the 1.0.3 upgrade file and unpacked it to the cmsmadesimple-1.0.2 root directory.  I can see the /install directory and upgrade.php file within that.  I've chmod'd the permissions all the way to 777 on the upgrade.php file (development server, behind a firewall) and the /install directory is 755.  Tried all different combinations of paths to get to upgrade.php in the browser and always receive a 404 error.  Tried creating symlinks inside public_html and admin folders to upgrade.php but no difference.  config.php doesn't reference the /install directory.  Should it?  Any ideas of what's going on?  This server was probably first installed with 1.0.2 and never had an upgrade.

Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request.
