formbuilder giving blank fields on site and in email

after filling out fields in formbuilder i get this page

Hi Chotaboy,

I don't know if you need it, but... If you have changed the name of the fields and did not in the template, the information will arrive empty. Example:

Original name:

Your Name
Your Email Address

Changed for:

Name:
Email:

If you didn't change in the template, the script look for "Your Name" and "Your Email Address" in GET or POST variable (I don't remember now) but this doesn't exist.

yes this works

after night after night of beating my head into a wall... XD

i saw a little tiny blurb about changing the template

and went

OMG <--- in a sarcastic / kill my self kinda way :)

works great now!
