SiteMapMadeSimple Installation Problem

I am trying to install the SiteMapMadeSimple 1.1.1 module on CMSMS 1.2.3 with CGExtensions 1.6.

When I click to install the module from Extensions -> Modules the screen submits and returns to Extensions => Modules with nothing to show, no errors, nothing.  Has anyone got any ideas?

Since posting I've attempted to view source of the page and set debug=true in config.php but nothing.

Interesting thing is when I set debug=true and attempt to post I get no debug back just the submitted page, admin menu  and blank page.

With debug true check, in html source, the last row and check your server log for errors
