Form builder: how to ensure success

Hi Guys,
I wanted to learn how to use the Form builder properly.
If I can come up with a simple "How to - for dummies" for creating a "single" text input field form step by step I will post a "How to - for dummies" in the wiki.

scenario:
Say I want to create a form with only one text field called: " Your name". purpose to generate an email to be sent to myadmin@.doman.com

What steps do I need to take to ensure that the form/email functions  and an e-mail is sent.

any one prepared to help?

criteria:
single text field labeled "Your Name"

email results to "myadmin@domain.com

Using all templates including. CSS classes etc.

Or
alternatively
a glossary of all tabs used in the module and what functions they perform and how to use them. [ for dummies ]
