Custom Form/mod for appointments

I have a client who has a service business that is performed on site either at the customers home or office. They need a form where current or new customers can schedule appointments. Idealy it would look/work something like this:

Returning Customers
  • email address field
(This would then take them to their account and would show past appointments and allow them to make a new appointment)
New Customers
Location: Residence
  • First Name
  • Last Name
  • Contact Phone
  • Street Address
  • Zip Code
  • Email Address
Location: Business
  • First Name
  • Last Name
  • Contact Phone
  • Street Address
  • Suite
  • Zip Code
  • Email Address
(Once they get enough clients at a particular business or building on a regular basis, there will be a drop down box with the business' name which will allow the program to show the client when the service will be at their building next. The above form fields could be just one with business or residential as check boxes)

Services:This would be a list of services provided with the price beside each one. It would be great if an estimate could be displayed on the confirmation page.
In the admin section it would idealy show my client a schedule and if there are any conflicts my client can contact the customer to reschedule.

I am comfortable setting up cmsmadesimple but I don't know enough programming to get this accomplished. If you are interested in this job, please send me an email with any questions or quote. My client needs this, of course, ASAP.

You can do much of this right now with the current SelfRegistration, FrontEndUsers, and CustomContent modules.  The scheduling/quote thing would be a seperate module of course.
