I have a client that uses a flash script for a photo album. It automatically pulls all image files in a given directory. Whenever someone mistakenly uses the image manager and looks at one of the directories that contains the images, the image manager always creates thumbnails in that directory. These new thumbnails then screw up the photo album and have to be removed.
If it's not too difficult to add somewhere, please consider coding an option where the image manager will not create thumbnails at all.
