Advanced Search module...

Hi, I'm trying to make a web site for a Physicians Association, and they gave me this site...

To work off of.  They want a similar advanced search feature.  Do any of you guys have any ideas as to what I should do to make something like tihs?  I plan on using the cataloger to make some physician bio pages with certain fields, name, address, practice, etc...

If anyone could help me, it would be ver apprieciated.  Something similiar to the link I posted, with dropdown field choices.

I don't think it's possible with official CMS MS modules. In your case I would write a module, specific for that case with it's own database table to keep all doctors and an actions to display and search for doctor.
