[solved] Menus and Pages disappearing after page permissions changes

Something is really amiss here and I've not had enough time to thoroughly document it but I wanted to "get it out there" as quickly as I could to see if any others are seeing this.

I've got a page named "Youth" in both Menu and Title. If I, the owner of the page, publish the page, it shows up in the Menu and on the site as you would expect. If I give the page an additional editor, and submit it again, the page becomes non-active and no longer shows up on the site. When I make the page active again, it never shows up again on the site either in the menu or anywhere.

This is very perplexing but perhaps part of it is an "undocumented feature"

I have Users assigned to a Group that only have privileges to edit a specific page. After I have added the page as the Admin and Owner, I add the specific user to be an additional editor and that user is part of the Group that only has permission to edit the page. Once the user has edited the page, the page options change so that it is hidden in menu and inactive. Is this by design so Admin can approve changes? If so, how am I do know what pages have been edited so I can turn the pages active and show in menu again? If not, why is this happening?
