Which feedback form works? [solved]

I have just tried my second Feedback Form module (v. 0.9.13) and got:
Fatal error: Call to a member function on a non-object in /home/cruicksh/public_html/modules/FeedbackForm/classes/TextInput.class.php on line 58
after adding a text field.

The first I tried wasn't compatible with my version of cms ms (v. 0.9.15). I'm using: CMS Made Simple 1.1.2 "Savaii"

I'm just looking for a simple spam proof contact from with Name, Email, Comment fields. Can someone tell me which one I should use?

as mentioned by cubix is FormBuilder the new module that replaces FeebbackForm. This module gives flexibility in creating your own forms and style them.

If you just want a contact-form, you can use the tag {contact_form} as well, which comes with the installation of the core. This one is to be found under "Extensions -> Tags".

Captcha can be turned on to be used for both.

your explanation was correct, I just wanted to say that for simple contactforms, the tag was available.
