Formbuilder Attachments?

I am running CMS 1.04 with Formbuilder 0.2.2.

I have read about the ability to have an attachment field in the form but I'm not seeing it.  Can anybody point it out to me?

Select "File Upload" as field-type for a new field in the definition of the form.

This is weird, but it's not showing in the list.  Does it need another module to work or should this do it?
