The only reason this happens if the 'INSERT' statement that inserts the list record into the database fails.
The only way this could happen (that I can think of). is that you upgraded from an earlier version of NMS. You may want to try uninstalling and re-installing.

...but now, when a subscriber gets in he receivers this confirmation mail:
