Hi,
I had a lovely form which worked very nicely (Form Builder 0.2 on CMSMS 1.0.3), but as soon as enabled mod_rewrite to use pretty URLs, it disappeared. Admin > Extensions > Form Builder tab is there but brings up a blank page!
It says "FormBuilder does not yet support pretty urls, although that shouldn't matter since the user side is pretty simple", but does seem to matter.
What do I do now? Should I uninstall the module and re-install again? Or is there a tweak that will make things re-appear again?

Can anyone at least tell me how I can contact the person who wrote the module. I am assuming it was not the people who scripted CMSMS.
Alternatively, can anyone from CMSMS advise the cleanest way to get rid of this module from the CMS installation - given that it is not visible in the CMS anymore.

I wish I could help you, but I don't use the Form Builder module. I can tell you that if you go into Extensions --> Modules, you will see all the modules currently installed. Click Uninstall, then if you want to remove it, click on remove. Otherwise, you might want to try to reinstall it with mod_rewrite on. That might even bring back your form... but don't quote me on that one!

Thanks for your suggestion. Eventually I managed to uninstall it.
After uninstalling via the CMS admin, I had to physically delete the FormBuilder directory and all its files from the server, then install a brand new one.
All a bit mysterious, but it is now visible. Will it work? Who knows! I'll get to it tomorrow.
