changes to contact form

Hi.

I've made some additions to the contact form, but seem to be stuck on how to add the following:
Information Requests
(check box) Yes  (check box) No  Please include me in your newsletter mailing
(check box) Yes  (check box) No  Please add me to your company mailing list.

If anyone can tell me the php coding I need to be able to add the above to the contact form it would be much appreciated.

Without PHP knowledge I guess it's easier to use the FeedbackForm module.
