Uploads Module Not Uploading When Used In Formbuilder

OK, so I have Uploads and Formbuilder working together and just want uploads to upload simple documents through the formbuilder, for some reason the attachments won't upload through the formbuilder form and put them into the allocated folder names "CVs"...

Now just using the uploads module and having the smarty {cms_module module="Uploads" category="CVs" action="upload"} works just fine, uploads the file perfectly etc.

I have this setup working perfectly on the previous website (we are currently updating their website to a new look and the new CMSMS 2 version) running CMS Made Simple™ 1.11.10 “Pinzon”, Uploads 1.17 and FormBuilder 0.7.4. I have replicated exactly the directories and all the preferences as of the last website, see screenshots below for the uploads options.
