Form builder: image within the radio button group

Hello everyone,

I am building a form with 40 questions. For each question there are 2 or 3 pictures and accordingly radio buttons A B or C, so that the user can choose one he likes best, and I get the results on e-mail. Getting the form to work is all done, but the last thing is, if it is possible and how do I add pictures in this form?

I have attached a picture of the approximate result I would need to have. Also, clicking on the images (as if they were radio buttons) would also be good, but I think it is easier to stick with normal radio buttons and images above them).

Any help would be great!
