Formbuilder Install Crashed Site

I was attempting to install current version of FormBuilder for a simple contact form for my site and had already installed and upgraded CMSMailer.

When I clicked on the Download & Install link for Formbuilder I got an error and can no longer see my site or even log into the CMSMS admin.

The error is:
Fatal error: Class 'formbuilder_utils' not found in /home/simonm/simonmetz.com/modules/FormBuilder/classes/Form.class.php on line 59

How can I correct the install or add the formbuilder utils or at least get into the admin section/

domain is simonmetz.com

Rename module folder /modules/formbuilder/ to i.e. /modules/_formbuilder_/

Thanks! got in through SSH

Is there a way to manually download the FormBuilder module and install via FTP, not sure why it crashed, I saw other posts which imply it might be memory issue in PHP?

Download latest xml file from
http://dev.cmsmadesimple.org/project/files/69

Then: Extensions > Modules > bottom of page - upload xml file. That uploads it. Then install it by clicking the link.
