FormBuilder - Submission Template Not Displaying

Didn't change any configuration.

FormBuilder just decided to stop displaying the submission template.

In its place, after a form is submitted, it renders the form page with CSS issues.

Tried to export and re-import. Only happens on 2 forms.

Need to figure out how to get it resolve.

Form page with css issues: I'd expect a incomplete rendered output (view page source).

There might occur a php error. Maybe the webserver error log shows something useful.
