admin not showing full admin menu

Sorry - I think I posted this in the wrong place for help - so let me try again.

When I log in admin menu is not showing full admin - just content/Extensions/preferences

I can't even add a new user

I went into phpMyAdmin - and the admin name is set with admin_access =1

so what happened to the rest of my menu

We are using a very old version 1.4.1

I'm not even sure where to begin upgrade !
