text missing on heading and menu tab in admin console

Hi,
The site is missing text that says "uploads" in the admin/content menu tab and on the header of the admin/uploads page. The tab is there on the menu itself and if you click on the blank space it does bring me to the admin/uploads page which is functioning normally. No error messages and nothing funky going on except I'm missing the menu tab text and header text. Never had a problem before I installed the avplayer and the youtubeplayer.
This had happened after I uninstalled the avplayer module and youtube module assuming it was the cause and hoping it would fix it but it hadn't so I then re-installed them.
I'm also attaching a print screen to show what I'm refferring too.

I'm running:
CMS Made Simple™ 1.11.9 “Bartolome”
Uploads version - 1.16.14
FrontEndUsers version - 1.22.1
avplayer version - 1.3.2
youtubeplayer - 1.3.1

Note: the space in between news and avplayer is the uploads tab.

All your modules and core are old versions. First update your CMSMS and modules to the newest version.

I realize it is 1 year old.
Does anyone know the file / file's that would be the source of the menu text?

I don't use Uploads module but with some modules there is an option to change their name - check if Uploads has this option and there is a blank field there in place of a name?

Check source of the admin page. Is there anything in the HTML in place of the name?
