Form Builder error messages

Running CMSMS 1.11.10. Latest version of Formbuilder module. Captcha not installed though this makes no difference to the error messages when it is.
On the page in which the Form is displayed I get the following error messages:

I have cleared the cache. PHP version 5.2.6

Try commenting out the captcha tags from the FB form template AND/OR be sure the Captcha option is not checkmarked in the form's Captcha settings.
