[SOLVED] FormBuilder not saving to ListItExtended instance

I have selected *Store results for ListItExtended and it worked fine, once. I deleted the item from within ListItExtended and now it whenever I fill in the form, it won't create a new item in ListItExtended. I get the email of the form, but not the ListItExtended item.

Sorted it out. There was no form input related to the "Title" of the item.
