Forgot password and Change username links not working in FEU

All of sudden, the forgot password and change username link are both not working on my site. It was working fine all along. Now when I click on both link, it took me direct to the homepage. I checked the html source, it seems the url is blank like this: <a href="" title="Click here if you cannot remember your password">Forgot Your Password?</a>.

My system info:
CMS Version 1.9.4.2
FrontEndUsers 1.15.1
FEU_Auth_Basic1.0
SelfRegistration1.6.12
Current PHP Version (phpversion): 5.2.17

I did not change anything from the admin side, it seems now the {$url_forgot} passes no variable.

This is what I saw when clicking on the forgotpassword link under the debug mode: notice:%20Undefined%20index:%20%20url_forgot%20in%20/hsphere/local/home/c30301/mysite.com/tmp/templates_c/FrontEndUsers^%%A2^A23^A2378C6E%%module_db_tpl%3AFrontEndUsers%3Bfeusers_loginform.php%20on%20line%2033

Update your cms and modules, they are way too old. 1.9.4.2 is no longer supported.

Note: latest releases of FEU needs PHP version 5.3+
