Form Builder 0.7.3 missing text input field?

Hello there.

I am building a pretty simple form with Form Builder 0.7.3, but get some pretty weird problems with missing text input fields.

Problem...

I create the first text input field identified as "Name", I click on the "Save and continue editing"-button, view it on the webpage and it shows up as expected.

I create one more text input field identified as "Address", I click on the "Save and continue editing"-button, view it on the webpage and it shows up as expected, but the first created text input field "Name" is now missing?

This is pretty simple, how can it go wrong?

I use CMSMS 1.11.3 “Darwin”
