The "Orders" form was working last week. I made some minor changes to it (replaced states with UK counties, and changed the template to add asterisks for required fields) and since then it hasn't worked. The page simply displays unformatted (no CSS) page template, and no content.
I have tried uninstalling, removing and then re-installing Orders (and FRTaxes, Promotions and DestinationBasedShipping as dependent modules), setting the Billing Form template to a single word, making a new page with the simple command {Orders} on it, even creating a new page template (with simply {content}) and setting the new page to that. Same problem every time.
You can see the site at http://www.spbeta1.me.uk - the checkout page is at http://www.spbeta1.me.uk/shop/checkout/details
Anonymous checkout is enabled, no address retreival method or other link to FEUsers, etc.
Help!

Re: 'Orders' not displaying billing form
it appears that orders does not currently work with the latest cgextensions.
http://dev.cmsmadesimple.org/bug/view/9122
and it may be that the latest FEU won't work without the latest cgextensions (even though the dependency is listed as cgextensions 1.31), at least that's my guess based on the fact that i had a fully updated install with broken orders page and working FEU pages but when trying to update excluding cgextensions i had broken FEU pages in admin and on front end. so i'm not seeing an upgrade path until either orders or cgextensions is updated. I would suspect that a new orders version is on the way as the current version is nearly a year old.

Re: 'Orders' not displaying billing form
I had to go nuclear - delete the entire install and start from scratch!

Re: 'Orders' not displaying billing form
i would be interested to know if you end up with a working Orders billing page with core, FEU and CGExtensions all at their latest versions. The nuclear option is not on the table for the site i'm having issues with at the moment.
Re: 'Orders' not displaying billing form
The same with me. Want to replace PPButtonMaker with Ecommerce.
CMS Version 1.11.4 or 5. on fresh install
Ecommerce Base
Cart
Order Manager
Paypal Gateway
FEU
The Orders-Page e.g. Checkout shows a Page without styling. Because all the <head></head> section in output is empty.
And with no content.
