FeedbackForm - file upload not working?

I'm using the FeedbackForm module 0.9.12 with CMSMS 0.13.  When I create a new form that includes a required file upload form field, the form always bounces back with a "Form field is required" message for the file upload field.

Any ideas on this?

Another piece of info:  if I make the Upload File field not-required, the form and file get submitted successfully.

Any ideas anyone?
