NOOB Needs Help Making Minor Mod to CMMS Form(s)

I have an existing CMMS site.. my orig developer is no longer available to me... and I'm stumped! I need help from someone that is very skilled in navigating around CMMS.

I have a simple Page Creation/Form Template set-up I need to do, but the last few tweaks are alluding me. I bet someone that knows what they're doing can knock this out in less than 1 hour.

What I'm doing is creating new pages from existing templates. These pages are used to create new landing pages. The landing page is for an EBook offer. If the visitor goes to the landing page and fills out the form and clicks "download now", the form spawns an email that sends the visitor a URL link that allows them to download the EBook. The form also spawns a second "thank you" page as well.

My confusion is this.. I've partially setup the new Landing Page, but for the life of me I can't figure out how to edit the forms in such a way that it's linked to the correct EBook PDF file that will be sent to the visitor if they fill out the form.
