Group Assignment/Permissions Disappear

I recently created a new user. After creating him, I realized I did not give him proper permissions. I changed his permissions and now my permissions table is completely blank. I am still able to function okay, but I wanted to find out if there is a way to fix the blank permissions issue.

It appears as though this was an issue in the past so the first thing I did was upgrade to the latest version 1.10.1. I was running 1.9.4.

I have upgraded my CMS a few times so I am not sure if there is some legacy stuff still in there that I need to clean out. I am not really sure how I would clean that out though.

Any assistance would be greatly appreciated.
