Survey Module

Hi, I hope I got this post in a reasonable board. If not my apologies.

I need to administer a few surveys. These surveys will be taken by representatives of various government offices that my employer has been contracted to gather data on. There will be about 100 responding offices and the data collected will be in the form of 'select a category' (only 1 best answer or all that apply), 'fill in a numeric value', and text.

I need a module or set of modules to provide a login (maybe not necessary but would be nice), to administer the survey and, to provide results in a usable format (spreadsheet or at least .cvs file).

I have read over the boards and I have seen a few different ways of doing this. So, I am asking for some advice on the best way (ease of implementation and of administration and some nice output format) of doing this.

Hi,

a starting point would probably be
FronteEndUsers (User login)
FormBuilder (to create your survey forms)
FormBrowser (to save submited results and export in csv)
