String(71) from Form Builder

Hi there.
When i try to use Form Builder ive get this wrong messenge!

string(71) "Smarty error: unable to read resource: "module_db_tpl:FormBuilder;fb_2""

What is wrong?
I have try everything but cant find somthing wrong.
Please help

Most possibly you have some typo in your FormBuilder Template.
