Gastbuch (Guestbook) Modification: add City and State fields to input form

I would like to add  "City" and "State/Province" fields to the Guestbook module.

Can anyone tell me how that could be done?

I'm not a programmer, but I can follow directions.

However, if someone wants to do the hack and provide me with the download that would be very generous and appreciated.
