It has to have 2 colums

Please make me an offer on how much i would spend for this.
I need only the Form-Template Layout, the rest i will do...

there is a very easy solution for this.
Use Class "right" in your advanced settings.
Do this for each field you wane line up to the right.
See the attachment jpg.

then make sure you have this code in your CSS file:
